- Collaborate with other professionals to evaluate patient's medical or physical condition and
to assess client needs.
- Utilize consultation data & social work experience to plan & coordinate client or patient care
& rehabilitation, following through to ensure service efficacy.
- Oversee Medicaid & Medicare related paperwork & recordkeeping in hospitals.
- Provide customer needs assessment, meeting quality standards for services & evaluation of
customer satisfaction.
- Perform other task that may be assigned by the superior.
_QUALIFICATIONS:_ - Graduated Bachelor of Science in Social Worker
Licensed Social Worker w/ updated PRC
