Job Description: The Patient Care Coordinator (Virtual Assistant, RN) is a key role responsible for enhancing patient care and ensuring a seamless healthcare experience in a remote capacity. This position requires a licensed Registered Nurse (RN) with exceptional clinical knowledge, organizational skills, and the ability to manage patient care activities virtually. Experience as a healthcare virtual assistant is a significant plus.
The Patient Care Coordinator will manage patient interactions, coordinate care activities, and provide education on treatment plans while collaborating with healthcare teams to ensure comprehensive patient care.
Provide virtual assistance to patients, addressing inquiries via phone, email, and video calls.
- Conduct initial patient assessments, including obtaining medical histories and reviewing current medications.
Offer compassionate support and guidance to patients and their families.
Schedule and manage patient appointments and follow-up visits, ensuring optimal timing for treatments and consultations.- Coordinate with healthcare providers to arrange necessary services such as lab tests, imaging, and specialist referrals.
Maintain and update patient records accurately and confidentially.
Serve as the primary liaison between patients, families, and healthcare providers in a virtual setting.
- Facilitate clear and effective communication within multidisciplinary teams to streamline patient care.
Provide detailed instructions to patients regarding treatment plans, medications, and follow-up care.
- 4. Administrative Duties:
Ensure patient confidentiality in accordance with HIPAA regulations.- Assist with virtual billing and insurance claims, ensuring proper documentation and timely submission.
Monitor and manage patient flow to minimize virtual wait times and optimize the patient experience.
Educate patients and their families about diagnoses, treatments, and care plans through virtual platforms.
- Provide resources and support for patients to manage their health effectively.
Organize and conduct virtual patient education workshops and support groups as needed.
Participate in quality improvement initiatives to enhance patient care and satisfaction.- Collect and analyze patient feedback to identify areas for improvement.
Implement and adhere to best practices in patient care coordination.
Bachelor of Science in Nursing (BSN) preferred.
- Current and valid Registered Nurse (RN) license.
Minimum of 3 years of nursing experience, with a preference for experience in patient care coordination.- Experience working as a virtual assistant in a healthcare setting is highly desirable.
