The Outpatient Medical Coder is responsible for reviewing and coding medical records for outpatient services. This role ensures that outpatient encounters, including office visits, diagnostic tests, and minor procedures, are accurately coded to reflect the services provided, supporting appropriate billing, compliance, and reimbursement.
Coding and Classification:
- Accurately assign ICD-10-CM codes for diagnoses and CPT/HCPCS codes for procedures based on outpatient medical records.
Ensure that coding reflects the complexity and nature of the outpatient services provided.- Follow coding guidelines and conventions to ensure compliance with regulations and accuracy.
Documentation Review:
- Examine patient charts, physician notes, and other medical documentation to determine the correct codes.
Ensure documentation supports the codes assigned and seek additional information or clarification from healthcare providers if needed.
