Job Description

General Descriptions:

Medical Records associate perform administrative tasks which require in maintaining the accuracy, privacy and confidentiality of patient's information. They are responsible in keeping a well organized patient's record and pertinent statistics.

  • Receptionist on Duty

  • Generate daily discharge report from the Bizbox System

  • Receive complete and incomplete patient's medical records from General Nursing Unit within the day until 4:00 PM. Those patients who discharge after 4:00 PM it should be brought by 9:00 AM the following day.

  • Segregate complete and incomplete patient's medical records based on daily monitoring.

  • Re-assemble and fill-out front unit sheet for each patient medical records

  • Answer telephone/walk-in patient's inquiry and other related concerns regarding medical records.

  • Facilitate incoming and outgoing patients request for medical records.

  • Prepare and photocopy all requested pertinent medical records of the patient after the approval of the approving officer. This is also include preparation of certified true copy of complete clinical record of the patient as per required of Philhealth Circular No. 2017-0028.

  • Receive late laboratory results and diagnostic procedures.

  • Receive patient medical records form ACC and encode to ACC logbook.

  • Call the attention of the Resident Physician In-Charge for immediate completion of Medical Abstract/Discharge Summary, History and Physical Examination due to patient's request.
  • B.2. Analysis of Chart

  • Analyze each patient medical records and assign the doctors and nurses deficiency using Analysis Sheet.

  • Register each patient's name against individual deficiency index card of doctors, general nursing unit and ancillary department.

  • Make monthly deficiency report for doctor, nurses, and ancillary. It must be reported to the office of the Medical Director, Chairman, Training Officer, and the Department Head/Division Group Head of their respective departments.

  • Folder each patient medical records and assign the appropriate numbers using terminal digit color code.

  • Prepare ad submit reported cases of Non-Communicable Diseases and Injuries to Department of Health as per Administrative Order No. 2013-0005.
  • B.3. Birth Certificate

  • Collect Newborn Data from the Delivery Room.

  • Make rounds to the patient's room to ask for the requirements needed to facilitate birth certificate and if it is available let the mother/father fill-out the birth certificate pre-form.

  • Type and prepare the data supplied by the informant (mother/father) on the final form of the birth certificate.

  • Return to patient's room and let the informant (mother/father) check/review the final form of the Birth Certificate.

  • Make rounds to Doctor's Clinic for the signature of the Attending Physician (Birth Certificate).

  • Re-checking of birth certificate and prepare transmittals and should be forwarded to Local Civil registry, Manila.

  • Attached the hospital copy of the Birth Certificate together with the pre-form to each newborn record.
  • B.4. Death / Fetal Certificate

  • Receive Death/Fetal and Disremembered Body Part Certificate from the Nursing Unit.

  • Type and prepare the data supplied on the final form (Certificate of Death/Fetal and Disremembered Body Part).

  • Verify the Status of account of the patient to Financial Counselor before the releasing of documents to patient's relative.

  • Attach hospital copy of death certificate together with the pre-form to each patient's medical records.

  • Make monthly mortality report.

    _Working Conditions_

    Can work 40 hours a week
    Can work under time pressure with minimum supervision.
    Performs other duties of personnel who are day off, absent, or on leave
    Such other duties as may from time to time may be prescribed by proper authorities.
    Know how to protect the privacy and confidentiality of patient's information.
  • Requirements

  • Graduate of any 4-year course.

  • Must have good communication skills in both Filipino and English

  • Computer literate

  • Able to handle people very calmly and effectively.

  • Problem solving and critical thinking skills.

  • With knowledge in keeping records.

  • Preferably with previous training in Health & Information pertaining to diagnosis, treatment, and end result.
