The Medical Coder is responsible for accurate data gathering for statistical analysis necessary for the mandatory reports to regulatory agencies, such as the PHIC, DOH and for research studies management from data collection, filing, storage, retrieval and disposal.
- Responsible for collecting and filing of discharged charts with 100% compliance on set standard time of collection and filing. Shall verify number of actual charts collected against daily discharge summary census. Ensuring 0% incidence of error and delay.
- Audits chart accurately with 100% compliance on identified deficiencies in patient's medical record chart. Ensuring 0% incidence of error and delay.
- Prepares monthly Chart Audit report every 10th of the month with analysis and recommendations to decrease the causes of deficiencies, if not totally eradicated with 100% accuracy of the reported data. Audit reports shall be provided to the management and quality assurance office to serve as reliable indicators in measuring Medical Records Department's compliance performance.
- Monitors and follow-up 100% compliance on completion of chart deficiencies of Attending Physicians, Nurses and Resident On Duty to hasten the processing of PhilHealth claims and ensuring 0% incidence of error and delay.
- Provides pertinent patient medical records (e.g. minor procedure sheet, OR sponge sheet etc.,) requested by PhilHealth Section as requirement to process PHIC claims of customers with 100% compliance on accurate documentations, ensuring 0% incidence of error and delay.
- Cleans daily, assemble and sort out all medical records of discharged patients daily with 100% compliance with the guidelines and minimum data sets, ensuring 0% incidence of error and delay.
- Orients new nurses regarding the minimum data set standard on documentation process while handling patients medical record chart and encoding of admitting and final diagnosis with 100% accuracy on proper documentation and recording. Ensuring 0% incidence of delay and error.
- Takes the minutes of the meeting during Departmental Monthly meeting with 0% incidence of error and delay.
- Exercises utmost tact, rapport and professionalism in dealing with consultants, co-employees, and the general public to ensure harmonious working relationship in rendering quality services attaining 100% customer satisfaction.
- Abides by all the established departmental policies with 0% incidence of deviations.
- Performs other duties as delegated by the superiors with 0% incidence of insubordination.
- Qualifications:
Must be a graduate of any four-year course, preferably with units in medical courses or anatomy and physiology;
- Must have attended training course related in Medical Records Management;
Must be a licensed ICD 10 Coder;
- Must have knowledge of medical terminologies and statistics;
Must have a good analytical skills;
