Responsibilities
- Conduct comprehensive assessments of patients to determine their physical, psychosocial, and emotional needs
- Coordinate the interdisciplinary team to develop individualized care plans based on patient assessments
- Monitor and document changes in patients conditions and update care plans accordingly
- Ensure accurate and timely completion of MDS assessments according to regulatory guidelines
- Collaborate with healthcare professionals to provide appropriate treatment and intervention plans for patients
- Assist in the training and education of staff on MDS assessment and care planning processes
- Utilize electronic medical records and other technology to efficiently manage and organize patient information
Requirements
- Bachelor's degree in Nursing or a related field
- Valid state RN license
- Minimum of 2 years of experience in a healthcare setting, preferably as an MDS Coordinator
- Strong understanding of MDS assessment process and Medicare/Medicaid reimbursement guidelines
- Knowledge of regulatory requirements and standards related to long-term care
- Excellent organizational and time management skills
- Ability to work effectively in a fast-paced environment
- Strong interpersonal and communication skills, both written and verbal
