The Intake Nurse is responsible for the initial assessment, prioritization, and coordination of patient care for patients based in the United States. This role involves collecting patient information, evaluating their medical condition, and determining the appropriate care pathway. The Intake Nurse acts as the first point of contact for patients and plays a crucial role in ensuring they receive timely and appropriate medical attention.
Responsibilities:
- Patient Assessment and Prioritization:
- Conduct thorough initial assessments of patients to determine their medical needs
- Collect and document patient information, including medical history, symptoms, and vital signs
- Use clinical judgment to prioritize patients based on the severity of their condition
- Coordination of Care:
- Coordinate with healthcare providers, including physicians, nurses, and specialists, to facilitate appropriate and timely care
- Ensure that all necessary documentation is completed accurately and promptly
- Communication:
- Serve as a liaison between patients and the healthcare team, providing clear and concise information
- Address patient inquiries and concerns with empathy and professionalism
- Patient Education and Support:
- Provide patients with information about the healthcare process and what to expect during their visit
- Answer patient questions and address concerns regarding administrative matters
- Data Management:
- Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system
- Generate reports and data analysis to support patient care and process improvement
Requirements
- Education: Bachelor of Science in Nursing (BSN)
- Licensure: Active Registered Nurse (RN) license in the Philippines
- Experience: Previous experience in intake, triage, emergency, or critical care nursing is preferred
- Skills:
- Strong clinical assessment and decision-making skills
- Strong communication and interpersonal skills
- Proficiency in using electronic medical record (EMR) systems
