The
Health Information System Form Builder/Document Control Officer will be responsible for designing, creating, reviewing, and maintaining manual/electronic forms used within the organization. The ideal candidate should have a strong understanding of hospital processes and has expertise on the hospital form design principles, excellent technical skills, and the ability to collaborate with various stakeholders to gather requirements and ensure that forms meet the needs of our healthcare professionals. _Applicants with experience of working with IQVIA / Arcus Air HIS is highly preferred. _
- Form Design and Development in Hospital Information System:
Design and develop electronic forms using designated software tools, specially IQVIA/Arcus Air, ensuring usability, accessibility, and compliance with industry standards.
- Collaborate with end-users, analysts, and stakeholders to gather requirements and understand workflow processes to translate them into effective form designs.
Implement form logic, validation rules, and data integrity checks to enhance form functionality and accuracy.- Customize and configure forms to integrate with existing hospital information systems and databases.
- Quality Assurance and Testing:
Review all the submitted HCCH forms and align requirements according to the approved policies.- Assign document code for each hospital forms.
Creates and manage hospital forms, policies, and other relevant documents in a secured and accessible database.
- Conduct thorough testing of forms to identify and resolve any issues related to functionality, usability, and data integrity.
Collaborate with Patient Safety and Quality Management Department to ensure that forms meet defined standards and requirements before deployment.- Perform user acceptance testing (UAT) and gather feedback for continuous improvement.
- Documentation and Training:
Provide training to end-users on form usage, best practices, and troubleshooting techniques.- Cascade any changes and updates to end-users
Maintain up-to-date documentation and knowledge base for all forms and related processes.
- Collaboration and Communication:
Works directly with Patient Safety and Quality Management Department, IT Department, Department/Unit Heads, and other Healthcare professionals, to understand requirements and deliver solutions.
- Communicate effectively with stakeholders to gather feedback, address concerns, and ensure alignment with project objectives and timelines.
Proactively identify opportunities for process improvement, automation, and optimization of form-related workflows.
Other ad hoc tasks or projects that may be assigned by immediate superior.

Requirements_
- Bachelor's degree in Computer Science, Information Systems, or related field.
Proven experience in designing and building electronic forms _within healthcare or related industries. _- Proficiency in form builder tools, specifically IQVIA or Arcus Air Hospital Information System
Strong understanding of form design principles, usability guidelines, and accessibility standards.
- Excellent analytical and problem-solving skills with a keen attention to detail.
Effective communication skills with the ability to collaborate across teams and communicate technical concepts to non-technical stakeholders.- Ability to work independently and manage multiple tasks in a fast-paced environment.
Certification in relevant technologies or methodologies _(e.g., Microsoft Power Platform, Agile) is preferred. _
