- Two (2) original copies of duly accomplished and notarized Personal Data Sheet (PDS) (CSC Form 212) with Work Experience Sheet (WES);
- Valid Certificate of Eligibility issued by the Civil Service Commission (CSC) and/or Valid License and Certificate of Registration issued by the Professional Regulation Commission (PRC)/Certificate of Bar Membership issued by the Supreme Court (SC);
- Photocopy of Official Transcript of Records (TOR) (Proof of meeting the Educational Requirement);
- Photocopy of Certificate of Trainings/Seminars attended (Proof of meeting the Training Requirement);
- Background Investigation Consent Form (HQP-HRF-182);
- Certification of Authenticity and Veracity of All Documents Submitted (HQP-HRF-165); and
- Performance Rating for the periods of January to June 2023 and July to December 2023:
- For government employees: Photocopy of Strategic Performance Management System (SPMS) rating
For external applicants with work experience: A Performance Rating Form (HQP-HRF-183) or any equivalent document providing the performance rating or evaluation issued by the last/current employer for the rating period, whichever is applicable.
