Key Responsibilities:- Assist clinicians in developing and implementing personalized care plans for patients, tailored to their individual health needs.
- Document patient vitals and health information accurately and efficiently, ensuring that all records are up to date and compliant with healthcare regulations.
- Educate patients about their diagnosis, treatment options, and care plans, providing ongoing support to encourage adherence and understanding.
- Communicate effectively with patientsu0027 physicians and other healthcare providers to ensure coordinated and comprehensive care.
- Provide 24/7 access to the Outreach Team for patient inquiries and support, fostering a reliable resource for patients when they need assistance.
Required Skills and Qualifications:- Strong communication and interpersonal skills to build rapport with patients and healthcare teams.
- Proficiency in documenting and managing patient health information, ensuring accuracy and confidentiality.
- Ability to educate and support patients in understanding their diagnosis and care plan, promoting self-management and empowerment.
- Excellent coordination and collaboration skills to work effectively with clinical and physician teams.
- Empathy and compassion towards patientsu0027 healthcare needs, recognizing the importance of emotional support in the healing process.
