JOB SUMMARYTo work in close coordination with the doctors and nurses ensuring the quality drugs are used appropriately for safe and effective medication for patient.
- DUTIES AND RESPONSIBILITIES
- Interview patient, family and other health care professional to complements patient's medical history, medication history.
Interprets doctor's order and accomplishes the Patient drug profile.
- Screens all referral request forms of prescribing restricted antibiotics.
Refers unavailable/ non- formulary drugs with available generic or therapeutic equivalents to the physician through the nurses.- Asses the status, etiology, risk factors and complications of the patient's medical problems.
Check's patient's medication card and medication box.
