Job Summary: We are looking to hire CNAs (Certified Nursing Assistant) and MAs ( Medical Assistant) And Nursing for RPM-Remote Patient Monitoring and CCM-Chronic Care Management
- Care Planning:
- Assess clients medical, social, and emotional needs to develop comprehensive and individualized care plans
- Work with clients, families, and healthcare professionals to establish care goals and coordinate care plans
- Coordination of Care:
- Coordinate and manage care services, including medical appointments, therapy sessions, and other supportive services
- Collaborate with medical professionals, therapists, and other care providers to ensure continuity and quality of care
- Monitoring and Evaluation:
- Monitor clients progress, track changes in their condition, and adjust care plans as needed
- Conduct regular evaluations and assessments to ensure care plans remain effective and relevant
- Advocacy and Support:
- Advocate for clients needs and preferences, ensuring they receive appropriate and respectful care
- Provide emotional support and guidance to clients and their families, helping them navigate care options and resources
- Documentation and Record-Keeping:
- Maintain accurate and up-to-date records of clients care plans, progress notes, and other relevant documentation
- Ensure compliance with regulatory requirements and standards for record-keeping and confidentiality
- Communication and Collaboration:
- Serve as a liaison between clients, families, healthcare providers, and support staff to facilitate effective communication
- Keep all stakeholders informed of clients care plans, progress, and any changes in their condition
- Resource Coordination:
- Connect clients and families with appropriate community resources, support groups, and services to enhance their quality of life
- Assist clients in accessing financial assistance programs and other resources as needed
- Crisis Intervention:
- Respond to emergency situations, assess risks, and take appropriate action to ensure clients safety and well-being
- Develop crisis intervention plans and work with healthcare professionals to address urgent needs
Qualifications:
- Bachelor's degree in Nursing, Social Work, Healthcare Administration, or related field required; advanced degree (e.g., MSN, MSW) is a plus
- Proven experience in care management, case management, or related roles in healthcare, social services, or long-term care
- Strong understanding of healthcare systems, insurance, and community resources
- Excellent communication and interpersonal skills, with the ability to build rapport and work collaboratively with diverse stakeholders
- Strong organizational skills and attention to detail, with the ability to manage multiple cases and prioritize tasks effectively
- Problem-solving skills and ability to navigate complex care situations
- Familiarity with healthcare regulations, standards, and best practices
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other relevant software applications
- Empathy, compassion, and a client-centered approach to care
- Integrity, reliability, and commitment to maintaining confidentiality of sensitive information
